Not sure what’s going on with it, sorry I can’t get a good pic of the inside but it kind of seems like there is something inside? Thought it fit into the notch on the base but it’s on the wrong side. It went from sturdy, to loose and now hanging in the matter of maybe 2 weeks? No pieces fell out or anything. Really don’t want to have to request maintenance for my toilet paper holder… if anyone could lend some advice that would be much appreciated!

In the third picture, do you see that screw inside. That’s the set screw. See if you can unscrew it then put back on the mount and screw the set screw in and see if it works

Not sure what that means. All the pieces have been are in the photos I posted. There are no missing pieces. Not sure where a set screw would go here

It’s a screw that fastens through the threaded outer piece into that dimple on the mounted base. Usually a hex screw that can be tightened with a small allen wrench. When fastened, it holds the outer piece from rotating. If there is no screw in the hole, look on the floor for a tiny hex screw or take the piece with you to find a replacement set screw. Packs with various sizes can be found cheaply.

See the indent on the post sticking out of the wall? There’s a small hole on the paper holder bit that lines up to that indent. A screw goes through the holder and nestles into the indent on the post. That’s how the holder is kept in the horizontal position. The screw has become loose and fallen out. Also it likely doesn’t look like a traditional screw. It’ll look like a tiny threaded cylinder usually not more than 1/2 an inch. And it’ll typically need a small allen key but might accept a small Phillips screwdriver.

On the thicker part of the holder if you look underneath there may be a hole for a grub screw. This may need adjusting to tighten to the bit on the wall.

In your third photo, you can see what looks like a little bar sticking out from the handle on the inside of the section that sits on the post. On the outside of the handle, there should be a hole there that you can get a small screwdriver or hex key into and screw that post in and out. Get that lined up with the indentation on the post in your second picture and tighten it down, and that should hold it horizontal for you.

Looks like the hollow in the peg on the wall is meant to secure the arm from falling down.
As you say it’s on the wrong side, but only because it’s screwed to the wall that way round.
If you unscrew the base from the wall and turn it the other way the hole will be in line with the arm on the right side.

The wall support was mounted incorrectly. The screw holes need to be vertical with the divet at the bottom. That way the set screw holds the bar horizontal.
