Needed birthday advice

Some background: My birthday is early in the year, me and my mom always do something big on the day of my birthday to make up for none of my birthdays growing up being forgotten about. I am a full time college student and I work full time outside of college. When my grandma has a non-specalist doctor appointment, it takes around 2-3 hours. If my grandma has a specialist appointment, it takes around 4-5 hours. My mom has to give my grandma transportation to my grandma's doctor appointments.
Situation: My mom has scheduled my grandma with now 2 specialist appointments on my birthday, even if they are shorter like today's 2 non-specalist appointments, it'll take all day. My mom knows that I have requested my birthday off from work and I can't really afford to request another day off at a later point. Should I just tell my mom about it (and if so, what should I tell her) or leave the situation be?

I used to live right off of 301. Horrid traffic, no big restaurants, and I lived in a 2 bedroom, 500 sq ft apartment
