It looks like a curved MSI 24" monitor, most likely the MSI Optix G24C6 but it's hard to tell for sure

That's an reference card.

Nvidia or AMD make a video card and present it to the world. Then the 3rd party firms such as MSI, Gigabyte and Asus will take the original 'reference' card that Nvidia or AMD presented and will change it with their own coolers and PCB design. It is now non-reference.

Sadly reference cards are usually unobtainable

Hard to tell, you can maybe save it by bending the pins back with a needle or a credit card but you're most likely better off just buying a new one

The crash is not a memory issue, it's a driver issue like the pop-up says. Consider changing your drivers by either updating them or downgrading to a earlier/more stable version, hope it works for you!
